Well established primary care group continues to experience growth and now has a need for an LCSW.
Participate in care coordination as member of a team of health professionals supporting the needs of patients in primary care
Engage and motivate patients to develop treatment options/goals that are congruent with identified psychosocial needs in collaboration with patient’s primary care provider.
